Why strength is likely the best indicator of aging

It predicts mortality better than most common markers—and better than muscle mass.

The question

It never comes up spontaneously. No one walks in asking to measure their grip strength.

However, everyone is interested in cholesterol, blood pressure, and weight. These are the markers we know, the ones a doctor checks, and the ones that appear on a blood test.

Strength doesn't appear anywhere in that picture. Yet, it is one of the most robust indicators we have.

What physiology tells us

The link to mortality is solid and consistent.

Analyses involving millions of people establish the same relationship: lower grip strength is associated with a higher risk of death, regardless of age, sex, or initial health status.

Every five-kilogram decrease corresponds to an approximately 16% increase in all-cause mortality risk, and even more for cardiovascular mortality. People in the bottom 20% have about twice the risk of those at the top of the distribution.

Strength predicts better than mass.

This is the most counterintuitive and important point. Having a lot of muscle isn't what protects you—it's being able to do something with it.

The literature is clear: muscle strength is a better predictor of mortality risk than muscle mass. Two people with the same amount of muscle can have very different health trajectories depending on their ability to produce force.

Why grip strength?

Because it is simple to measure and serves as a reliable proxy for overall health. Hand strength is strongly correlated with the strength of other major muscle groups, making it a practical summary of your total body strength.

What it really measures is your ability to remain independent. When strength declines, physical activity decreases, which impairs cardiovascular health, promotes insulin resistance, and increases the risk of falls. Weakness is not just a symptom of aging; it is a driver of it.

And unlike age, it is modifiable. This is what sets this marker apart from most others: strength responds to training at any age, even in the very elderly.

A balance sheet tells you what a company owns. It does not tell you what it is capable of producing.

A company can hold significant assets and be unable to fulfill an order. Another, leaner company can execute perfectly. When you evaluate its five-year viability, it is the latter information that matters.

Muscle mass is the balance sheet. Strength is the production capacity.

What we see at the studio

People arrive with weight goals and leave with strength goals. This shift is almost universal, and it usually takes a year.

What triggers it is not an argument. It is a concrete experience: carrying groceries without thinking about it, getting up from the floor without using your hands, climbing three flights of stairs without stopping. Things that were not goals at the start become non-negotiable once you have them.

We also see the flip side. People who arrive at fifty-five having lost the ability to stand up from a low chair without using their arms. They hadn't realized it: the loss was gradual, and they adapted their movements without noticing.

That is what makes the measurement useful. It makes visible what daily compensations hide.

No one wakes up one morning having lost their independence. You lose it one movement at a time, without seeing it happen.

What this changes for you

Measure your strength, not just your weight. It is a more informative data point regarding your long-term trajectory.

Track the progress, not the absolute value. What matters is the direction you are heading, not your position relative to anyone else.

Strength training is the only lever that directly impacts it. Cardio protects your heart and sleep protects your recovery, but neither builds strength.

It is never too late to take action. Strength responds to training at any age, even in people over ninety.

Do not confuse this marker with a diagnosis. Low strength is a statistical signal at the population level, not an individual prognosis. For any health-related questions, your doctor has the final say.

We do not measure aging in years. We measure it by what we are still able to do.
